Using FairPlay Streaming (FPS) technology, content providers, encoding vendors, and delivery networks can encrypt content, securely exchange keys, and protect playback on iOS.

1. Name : Name as given for Apple developer account (This will be auto fetched).
2. Email : Email-id of the Developer account (This will be auto fetched).
3. Team : As mentioned for Developer account (This will be auto fetched).
4. Website : Mention the organization website.
5. Streaming Distribution Partner Name : optional
6. Streaming Distribution Partner Website : optional
7. Is there a working FPS development server in which to use the FPS credentials? : Yes
8. Your Content : (Describe your content, who the content owners are, and why you want to protect playback with FPS) Here we need to describe it in proper English and must be described well
9. Your Company : (Describe what your company does and the business relationship you have with the owners of the content you will be streaming) Here we need to describe it in proper English and must be described well
